How to create and send forms to your clients

Forms are a great way to offer an even more personalized service to your clients! Here is how to use them.

Our digital forms are an easy way for you to obtain information and store information about your clients and to offer an even more personalized service to them!

Here's how to use them:


1. Accessing the Forms Feature

  1. Log in to your Salonkee account.

  2. Once logged in, go to the Campaigns section. Please note that this feature is only available for clients with the Business pack.

  3. If you are not a Business client yet, our Customer Success team will assist you with its activation.

  4. In the left-hand menu, you will find the Forms option. Click on it to access the forms feature.

2. Creating a New Form

  • To create a new form, click on the New form button.

  • Give your form a name, for example, "Health Questionnaire".

  • You can also write a brief description that clients will see when they fill out the form.

Sharing Settings (Salon Groups Only):


By choosing the All salons in the group option, you authorize the modification of the form and allow client responses to be viewed.

⚠️ Warning: Granting visibility access to responses is irreversible. Once granted, you cannot revoke it.

Form Configuration Settings


You have three options available:

  • Allow clients to edit answers: Activate this option if you want clients to be able to edit their answers after submitting the form.

  • Allow salon to edit answers: Activate this option if you want your salon staff to be able to edit client answers after submission.

  • Digital signature required: Activate this option if you want the client to digitally sign the form once completed. Note: This option makes the form responses non-editable in the future.

Adding Questions

Choose the appropriate question type from the available options by clicking "Add question":

  • Short text

  • Long text

  • Single choice

  • Multiple choice

  • Date

  • Description (useful for dividing the form into sections with a description at the beginning of each new section)

Add your questions and response options accordingly.
For example: "Do you have any allergies?

  • "Do you take prescription medication?" (Single choice: Yes or No)

  • "Do you have any allergies?" (Multiple choice: Latex, eggs, nuts, shellfish, and others [enter custom text])

  • "Describe how you want your hair done" (Long text)

  • "What is your date of birth?" (Date)

You can mark certain questions as Must answer if you need a mandatory answer for them

Optionally, you can choose to Show in booking for certain questions. This is useful for important information your salon needs to know before providing the service.

3. Previewing and Saving the Form

  • Once you are satisfied with the form configuration, click the Preview button to see what the form will look like to clients when they answer it.

  • After reviewing the form, click the Save button to save your form

4. Sending the Form to Clients


There are several ways to send the form to clients:

  • Select a client from your client list, go to the Forms tab, and click the New form formulaire button to send the form via email.

  • You can also provide the client with a QR code, allowing them to fill out the form on their mobile device directly at the salon.

  • Alternatively, the client can fill out the form directly on your computer.

You can also set up automated emails to automatically send forms to clients based on their reservation type or other criteria.

5. Viewing Form Responses

  • To view client responses to the forms, go to the Forms section.

  • Click on the specific form whose responses you wish to view.

  • You will see a list of responses along with the completion date and time.

  • If any changes are made to a client's responses, the form history will display these modifications

6. Managing Forms

In the forms section, you can perform various actions on each form:

  • Edit the form if you need to make changes.

  • Duplicate a form to create a copy with similar questions.

  • Delete a form if you no longer need it. Please note that once a form is deleted, clients can no longer respond to invitations to fill it out
